Elevated hematocrit — the percentage of blood volume occupied by red blood cells — is the most common lab abnormality on TRT. Testosterone stimulates erythropoiesis (red blood cell production) through EPO upregulation. When hematocrit exceeds 52–54%, blood viscosity increases and cardiovascular risk may elevate. The standard recommendation — "go donate blood" — is valid but shouldn't be the only tool in the kit.
The Full Toolkit
1. Dose Reduction
Higher testosterone doses produce more erythropoietic stimulation. A 20–25mg/week dose reduction often brings hematocrit down 1–2 points over 4–6 weeks. This is the first-line intervention — reduce the stimulus before managing the consequence.
2. Injection Frequency
Peak testosterone levels drive peak EPO stimulation. Switching from weekly to twice-weekly or daily injection lowers peaks while maintaining the same total weekly dose. Some men see meaningful hematocrit reduction from frequency change alone.
3. Subcutaneous Injection
SubQ injection produces lower peak testosterone than IM at the same dose. The lower peaks may reduce EPO stimulation. Evidence is mixed but the trend favors SubQ for hematocrit-sensitive men.
4. Hydration
Dehydration concentrates blood and artificially elevates hematocrit. A man who's mildly dehydrated at the time of a blood draw may show 52% when his true hydrated hematocrit is 49%. Consistent hydration (minimum 2.5L/day for active men) is the simplest lever.
5. Therapeutic Phlebotomy or Blood Donation
When other measures are insufficient, removing blood directly reduces hematocrit. A standard blood donation (500mL) reduces hematocrit by approximately 3 points. Therapeutic phlebotomy prescribed by your provider allows more frequent draws than standard donation centers permit. The main limitation of frequent phlebotomy: iron depletion, which causes fatigue — ironically mimicking the low-T symptoms you're treating.
6. Naringin (Grapefruit Extract)
Naringin, the flavonoid in grapefruit, has documented effects on red blood cell turnover. Some TRT clinicians recommend 500–1000mg naringin daily as a supplementary hematocrit management tool. The evidence is limited to preclinical studies, but the safety profile is favorable and anecdotal reports from the TRT community are positive.
Important: Grapefruit/naringin interacts with numerous medications by inhibiting CYP3A4 enzyme. Check for interactions with any other medications before adding naringin supplementation.

- Step 1: Ensure adequate hydration before retesting
