The Short Answer
Key Takeaway
Clomiphene (Clomid) is a mix of two isomers: enclomiphene, credited with most of the testosterone-raising effect, and zuclomiphene, which lingers in the body for weeks. Enclomiphene is the trans isomer on its own. Generic clomiphene is FDA-approved (for women), fillable at any pharmacy and can cost less; enclomiphene is compounded only and not FDA-approved. The only head-to-head data we found in men are retrospective, and they suggest similar testosterone gains with fewer side effects on enclomiphene.
One Drug, Two Isomers: What Clomid Actually Contains
Clomiphene citrate isn't a single molecule. The Clomid label describes it as a mixture of two geometric isomers, cis (zuclomiphene) and trans (enclomiphene), containing 30% to 50% of the cis isomer. FDA's 2022 review puts enclomiphene at about 50% to 70% of clomiphene citrate, per the USP monograph.
Both isomers are selective estrogen receptor modulators (SERMs). The case for isolating enclomiphene is that it does the useful work: it blocks estrogen's feedback signal, so the pituitary releases more LH and FSH, which drive testosterone production. FDA's briefing document summarizes the published view that zuclomiphene "contributes little to the intended outcomes" and has estrogen-agonist effects, while noting its effects are "not fully understood."
| Clomiphene (Clomid, generics) | Enclomiphene | |
|---|---|---|
| What it is | Mixture of zuclomiphene (30–50%) and enclomiphene | The trans isomer (enclomiphene) alone |
| FDA status | Approved for ovulatory dysfunction in women; use in men is off-label | Not approved for any use; FDA complete response letter in 2015, EMA refusal in 2018 |
| How you get it | Generic tablets from retail pharmacies, with a prescription | Compounded only, with a prescription, typically through telehealth clinics |
| Strengths | 50 mg tablets | 12.5 mg and 25 mg studied in Phase 3; TRT Nation lists 25 mg capsules |
| Half-life | Zuclomiphene detectable for more than a month after a dose | About 10 hours |
| Cash price (Oct. 2026) | 30 × 50 mg: about $43 to $168 (coupon estimates; varies by pharmacy and location) | About $99 to $199.99 a month at clinics we checked |
Half-Life: Hours Versus Weeks
This is the clearest difference between the two. FDA's review gives enclomiphene a half-life of about 10 hours. The Clomid label says zuclomiphene has a longer half-life than enclomiphene, and that in single-dose studies detectable zuclomiphene "persisted for longer than a month."
In men taking clomiphene long term, that adds up. A 2017 study of 15 men on clomiphene 25 mg daily, with a median of 25.9 months of treatment, found median blood levels of 44.0 ng/mL zuclomiphene versus 2.2 ng/mL enclomiphene, a ratio of about 20 to 1 (Helo et al., BJU International). Men on Clomid for months mostly carry the isomer that FDA's review says contributes little to the testosterone effect.
No trial has settled whether that buildup harms men. It's the main argument for enclomiphene. It also means switching isn't instant: zuclomiphene from earlier clomiphene can stay in your system for weeks after you stop.
What the Evidence Shows
Both raise testosterone
The best pooled data combine the two drugs. A 2025 meta-analysis of 10 randomized trials with 819 men found that clomiphene or enclomiphene raised total testosterone by about 274 ng/dL more than placebo, along with LH and FSH (Hohl et al., Archives of Endocrinology and Metabolism). Results varied widely between trials (I-squared 89%), and sperm concentration did not differ significantly from placebo. Compared with testosterone gel, total testosterone was statistically similar, but SERMs produced higher LH, FSH and sperm concentration. Symptom results were mixed, and the authors rated the placebo comparisons as moderate-certainty evidence.
Clomiphene has longer follow-up
Clomiphene has been prescribed off-label to men for years. In one small study of 46 men followed for three years, mean testosterone rose from 228 ng/dL at baseline to 612, 562 and 582 ng/dL at years one, two and three, and no adverse events were reported (Moskovic et al., BJU International, 2012).
Enclomiphene has company trials but no approval
Enclomiphene's two Phase 3 trials ran 16 weeks. They showed higher testosterone, LH and FSH with sperm concentration maintained, while testosterone gel sharply reduced sperm production (Kim et al., BJU International). Regulators weren't convinced a better lab number meant better health. FDA's 2022 review said serum testosterone is not a validated surrogate endpoint and found insufficient evidence of effectiveness, and the EMA noted the studies didn't test whether symptoms improved.
Head-to-head data are thin
We found no randomized trial that compares the two drugs directly in men with low testosterone. Two retrospective studies come closest:
- Saffati et al., 2024: 66 men at Baylor College of Medicine who took clomiphene and later switched to enclomiphene. Median testosterone rose 166 ng/dL on enclomiphene vs. 98 ng/dL on clomiphene, a difference that was not statistically significant (P=0.20). Adverse effects were reported by 47% of men on clomiphene vs. 13.8% on enclomiphene (Translational Andrology and Urology). There was no washout between drugs, so leftover clomiphene could have affected the enclomiphene results.
- Thomas et al., 2023: 46 men on enclomiphene and 32 on clomiphene, treated for infertility, abnormal semen parameters or hypogonadism. Both raised total testosterone, neither significantly changed semen volume or concentration, and enclomiphene did better at raising gonadotropins and total motile sperm count (Cureus).
Side Effects: Where the Two Differ
In the Saffati comparison, the gap showed up in how men felt. On clomiphene vs. enclomiphene, decreased libido was reported by 33.3% vs. 8.6%, decreased energy by 16.7% vs. 5.2%, and mood changes by 9.1% vs. 0%. Estradiol changed by +17.5 pg/mL on clomiphene and by −5.92 pg/mL on enclomiphene. Treat these as signals rather than settled rates: the study was retrospective, the same men took the drugs in sequence, and no one was blinded.
Other points worth knowing:
- Vision. The Clomid label warns that blurring or other visual symptoms such as spots or flashes may occur, and says anyone with visual symptoms should stop treatment and get a prompt eye evaluation. For enclomiphene, cataracts appeared in a dog toxicology study; the EMA found no conclusive evidence of a link in people but recommended ocular safety monitoring, according to FDA's review.
- Reports in men. The Clomid label notes that gynecomastia and testicular tumors have been reported in men using clomiphene, and that no adequate, well-controlled studies show it works for male infertility.
- Blood clots and blood counts. The EMA cited a risk of venous thromboembolism when it refused enclomiphene in 2018. FDA's review flagged cardiac and thromboembolic events, plus rises in estradiol, PSA and hematocrit.
- Drug testing. Clomiphene is on the WADA Prohibited List under Section 4, and enclomiphene falls under it too as a component of clomiphene, per the Defense Department's Operation Supplement Safety. See our guide to testing and documentation for athletes.

Cost and Access in 2026
Clomiphene is a generic, FDA-approved drug (for ovulatory dysfunction in women), so retail pharmacies can fill an off-label prescription written for a man. WebMDRx's coupon estimates for 30 tablets of 50 mg ranged from $43.10 at Walmart to $167.99 at Walgreens when we checked on October 12, 2026. These are estimates that vary by location and date, so shop around. How long 30 tablets last depends on the dose and schedule your prescriber sets, which is why it's hard to compare per month.
Enclomiphene has no approved version: FDA issued a complete response letter in 2015, and the EMA refused authorization in 2018. In the US, it's compounded. FDA's bulk-substance list, updated May 14, 2026, places enclomiphene citrate in Category 1, "under evaluation", where FDA says it doesn't intend to take action against 503A compounding as long as its other conditions are met. That status is provisional: in 2022, FDA proposed against adding enclomiphene to the 503A bulks list, and its advisory committee agreed 8 to 4.
Online prices we verified on October 12, 2026: TRT Nation lists enclomiphene at $99.99 a month for 30 × 25 mg capsules. Maximus lists $99.99 to $199.99 a month depending on plan length, and Hims starts at $99 a month on a 10-month plan paid upfront. Labs are generally extra. TRT Nation lists enclomiphene but not clomiphene. Our enclomiphene cost breakdown covers fees and plan terms in detail.
How to Choose, With Your Prescriber
Both are prescription decisions, and neither fits everyone. These are reasonable ways to frame the conversation:
| If this describes you | Worth asking about | Why |
|---|---|---|
| Cost is the deciding factor | Generic clomiphene | Pharmacy-dispensed FDA-approved product (used off-label), with coupon estimates from about $43 for 30 tablets at some pharmacies |
| You had libido, mood or estradiol problems on clomiphene | Enclomiphene | The only comparison data, though retrospective, found fewer side effects after switching |
| You want to avoid a long-lingering isomer | Enclomiphene | About a 10-hour half-life, vs. zuclomiphene detectable for over a month |
| You want a non-compounded, FDA-approved product | Clomiphene (off-label) or FDA-approved testosterone | No enclomiphene product is approved |
| You compete in drug-tested sports | Neither, without clearance | Both fall under WADA Section 4 |
Both drugs work by raising LH and FSH, so they depend on testes that can respond. Enclomiphene was studied, and evaluated by FDA for compounding, specifically in secondary hypogonadism; our guide to secondary vs. primary hypogonadism explains the difference. If fertility is the main goal, read our fertility and sperm preservation guide. For how either compares with testosterone itself, see enclomiphene vs. TRT.
Switching: Because zuclomiphene can persist for more than a month, past clomiphene may still be acting when you start enclomiphene. Ask your prescriber when to recheck labs after a switch, so the new numbers reflect the new drug.
The Bottom Line
Enclomiphene is the part of clomiphene thought to do most of the testosterone work, without the long-lived zuclomiphene. That's a real pharmacological difference, and early retrospective data hint it may mean fewer side effects. But it comes as a compounded, unapproved drug, usually at a higher monthly price than the lowest generic clomiphene coupon prices, and no randomized trial has compared the two directly.
If you're starting fresh and cost matters, ask your prescriber whether generic clomiphene is reasonable for you. If you tried clomiphene and felt worse, enclomiphene is the logical next question. Either way, get a proper baseline panel first and a recheck on the schedule your prescriber sets.
Frequently Asked Questions
Is enclomiphene just purified Clomid?
Essentially, yes. Enclomiphene is the trans isomer that makes up roughly 50% to 70% of clomiphene citrate; Clomid also contains 30% to 50% zuclomiphene. As a medicine, though, enclomiphene is not FDA-approved and is only available compounded in the US.
Which one raises testosterone more?
We found no randomized head-to-head trial. In a retrospective study of 66 men who switched from clomiphene to enclomiphene, median increases were 166 vs. 98 ng/dL, a difference that was not statistically significant.
Is clomiphene FDA-approved for men?
No. Clomiphene is FDA-approved for ovulatory dysfunction in women, so prescribing it for men is off-label. Its label says there are no adequate, well-controlled studies showing it works for male infertility.
Why does zuclomiphene matter?
It has a much longer half-life than enclomiphene, and the Clomid label says it was detectable for more than a month after a single dose. In men on long-term clomiphene, blood levels of zuclomiphene were about 20 times higher than enclomiphene.
Will clomiphene or enclomiphene cause problems with sports drug testing?
Yes, both can. Clomiphene is on the WADA Prohibited List under Section 4, and enclomiphene falls under the same section as a component of clomiphene.
