The most extensive long-term TRT data comes from European registry studies, particularly German observational cohorts that have followed hypogonadal men on TRT for 10+ years:
- Body composition: Waist circumference continued to decrease over 10 years. Weight loss was sustained. Lean mass gains plateaued by year 2–3 but were maintained long-term.
- Metabolic markers: HbA1c improvements sustained. Some men discontinued diabetes medications. Lipid profiles remained stable or improved.
- Cardiovascular: No increase in major cardiovascular events over 10+ years of observation. Some registries showed reduced cardiovascular mortality in the testosterone-treated group vs. untreated hypogonadal men (though observational data has inherent confounders).
- Prostate: PSA levels remained stable after the initial 6–12 month adjustment. No excess prostate cancer incidence in 10-year follow-up.
- Mortality: Multiple registries showed lower all-cause mortality in treated vs. untreated hypogonadal men over 10+ years of follow-up. This is consistent but cannot prove causation.
