Blood pressure gets less attention in TRT discussions than hematocrit or estradiol — but it deserves monitoring. Testosterone has documented effects on fluid retention, vascular tone, and renal sodium handling that can modestly increase blood pressure in susceptible men.
The Mechanism
Testosterone promotes sodium and water retention through direct renal effects and through estradiol (which also promotes fluid retention). The result: 1–3 kg of water weight gain in the first weeks, which can translate to 3–8 mmHg systolic blood pressure increase. For men with normal baseline blood pressure, this is clinically insignificant. For men with pre-existing borderline or uncontrolled hypertension, this modest increase can push values into a range that warrants intervention.
Monitoring Protocol

- Baseline: Documented BP before starting TRT
- Month 1: Recheck. If elevated >5 mmHg systolic, monitor weekly for trending.
- Quarterly: At every lab follow-up. Ideally home monitoring with a validated cuff.
- Target: <130/80 per ACC/AHA guidelines (same as general population)
- Intervention: If BP rises above target — evaluate dose, hydration, sodium intake, and consider antihypertensive if lifestyle measures are insufficient

Simple to Monitor, Easy to Miss
Blood pressure monitoring on TRT is cheap, painless, and clinically important. A home BP cuff ($25–$50) and weekly measurements catch trends early. Most men see no significant change. Those who do can address it through dose adjustment, sodium restriction, or — if needed — the same antihypertensives they'd use regardless of TRT. The key is measuring it in the first place.
